Application DS 11

• Applications can be obtained online on the U.S. State Department​ website.
• Applications should be completed prior to coming in but not signed; the application must be signed in front of a Passport Agent.
• All applications must be printed by hand in black ink or filled out online and printed.

Required Documents

• Bring the following documents with you for your appointment:
  • Application DS 11 (completed but not signed)
  • Proof of U.S. citizenship, including (but not limited to):
    • Long-form U.S. birth certificate from the Vital Records department of one's state of birth (e.g. Virginia Office of Vital Records) containing the full name(s) of the parent(s)
    • Original Certificate of Naturalization or Citizenship
    • Undamaged U.S. passport; may be expired.
    • Bring the original copy and a photocopy of the document(s) you use. Please see Travel.State.Gov for more information about IDs.
  • Proof of identity (government-issued photo ID)
    • Virginia (or border state): valid driver’s license
    • Military ID
    • Out of state identification (non-border state), then you must present two forms of photo identification.
  • Please see the U.S. Department of State’s website for acceptable identification.
  • One check or money order made payable to U.S. State Department
  • One check or money order made payable to the City of Virginia Beach

The City Clerk's Office offers to take photos on-site. However, you may also opt to bring a recent color photograph (according to photo requirements on travel.state.gov/photos).

Proof of Parental Relationship (for Children Under 16)

• Both parents/legal guardians must accompany the child and show the child's birth certificate and the parents'/guardians' proof of identity (original picture ID and a photocopy of the front and back); or
• The child and the appearing parent/guardian must bring one of the following:
  • Notarized Absent Parent Form DS3053 signed by the second parent (one form per child) and a front-and-back photocopy of the absent parent's picture ID
  • The second parent's/guardian's death certificate (if the second parent/guardian is deceased)
  • Evidence of sole authority to apply
  • Form DS-5525 explaining, in detail, why the second parent cannot be reached

Passport Renewals

• Adult renewals on a passport less than 15 years old self-renew by mailing in form DS82 (and its subsequent required information) along with payment for a regular or expedited renewal.
• If you acquired your passport when you were under 16, you must renew in person with form DS11 and the other above requirements.

A child under the age of 16 must appear in person with both parents (or guardians) to submit Form DS-11: Application for a U.S. Passport in person.

Both parents or legal guardians must do all of the following:

• ​Submit evidence of child's U.S. citizenship
• Submit evidence of relationship between child and parent(s)/guardian(s)
• Present identification of parent(s)/guardian(s)
• Submit a photocopy of each parent's/guardian's ID document
• Provide parental consent

If the second parent is not available to sign, the appearing parent must present evidence that he or she has:

• Sole legal custody of the child or
• Written consent of the non-appearing parent for the issuance of a passport.

The Children's Passport Issuance Alert Program (CPIAP) is one of the Department of State's most important tools for preventing international parental child abduction. 

The program allows parents to register their U.S. citizen children under the age of 18 in the Lookout System. If a passport application is submitted for a child who is registered in CPIAP, the Department of State contacts and alerts the parent or parents.
